Contact Info block
Use this block to get the contact information of the user (e.g., first name, last name, email, phone number, and company). All the field names are editable.
This block can also be used to get multiple inputs or add multiple questions in a single screen.
Short Text
Use this for short answers, such as Name or any other short text answer.
Long text
Use this if you want people to write longer answers.
For e.g User Feedback, Full address etc.
Email block
Use this block to ask for email.
You can use restrict the user to enter only business emails.
You can also use verification feature .
Phone number
Use this block to ask for phone number.
Please note that the country in the dial code is auto selected based on the user's browser settings. If your user is located in say New York, the dial code will auto-select the US flag for them.
Statement block
Primarily used as "Intro" or "Welcome" block, but can be used anywhere in your form to display a message.
Number
Ask users to enter a numeric value.
Website URL
Ask users to enter a website URL for e.g their LinkedIn profile or their portfolio website.
Single select option
Single Select Option blocks are used in questions where multiple options are presented to users but they can choose only one out of many.
Multi select option
Unlike Single Select Option blocks, these blocks are used in questions where multiple options are presented to users and they can choose more than one among them.
Dropdown
Use this block when you have large list of options to choose from. For example: What state do you live in?
Date
Allows users to select a date.
Address
Ask address of users: there are multiple fields like address line 1 and address line 2, city/state/zip code, and country. you can have an option to hide any of the fields or ask all of them.
Scheduler
Use this block to schedule appointments or meetings via Calendly, Cal.com or SavvyCal.
Star rating
Ask users for a star rating about your product or service.
Opinion scale
Ask users to express their opinion on a numbered scale and create NPS forms.
Signature
Ask users to e-sign. Please consult your local laws regarding e-signatures.
File upload or attachment block
Use this block to ask user to upload file(s). For example, ask for resume in a job application form. Free accounts have a 10MB per file limit, which is removed in the Pro plan.
Payment
Use this block to accept payments. Currently only Stripe is supported. This feature is only available in Youform Pro.
Ranking
Use this block to ask users to rank options.
Matrix
Use this block to ask users to rate on multiple parameters on the same page.
